If you already have a patient portal account and are trying to register another client (such as a family member), please log in to your portal, go to Linked Profiles, and select "Register Someone Else." Please make sure to complete all 3 steps of the registration process to successfully submit the form. If you are registering multiple family members for services, we recommend starting with one child completing a comprehensive psychological assessment. This allows us to better understand the family’s needs and inform individualized treatment planning. Following the assessment, the clinician will have collaborative conversations with caregivers regarding treatment recommendations at the family level. At that time, the clinician can submit an internal referral to connect additional children in the family with therapy services at UCCF.
